Therapeutic Services Agreement
At Nursing Essentially, we require clients who engage therapy to sign a Therapeutic Services Agreement. This document available for download here provides all information about what to expect during these session, prices, insurance details and at the end, requeires you to provide information about yourself, what you would like to achieve during therapy in addition to certain waivers of liability – particularly when engaging in Animal Assisted Therapy. This is also why a Confidentiality Agreement is also required which also ensure confidentiality is maintained (see below). This Agreement must be completed and submitted to Nursing Essentially prior to commencing any session with Nursing Essentially and can be emailed.
Confidentiality Agreement
At Nursing Essentially, we require clients who engage our services to sign a Confidentiality Agreement. This contract ensures that all information disclosed during sessions is held with the strictest of confidence (with the exception of legislative obligations required of Registered Nurses). This Agreement is available here, and will be emailed to you prior to commencing any session with Nursing Essentially. This agreement must be signed and Nursing Essentially will not provide any services to any client should this agreement not be signed, or, if the agreement is breached.
